STEP-BY-STEP PRE-REGISTRATION PROCEDURE FOR QUALIFIED STUDENT-APPLICANTS SY 2024-2025

STEP 1: Access the Google Form links to your respective colleges.

For Qualified Student-Applicants at the following colleges, kindly refer to the Google Form links provided below for your pre-registration:

STEP 2: Fill out your necessary data.

Upon accessing the link from your college to where you qualified, kindly fill out the data provided accurately.

Name of Student:

Write your full name in capital letters in this format.

(SURNAME, FIRST NAME M.I.)

Example: DELA CRUZ, JUAN B.

Program Applied:

Select your first-choice program to where you qualified.

Example: Bachelor of Science in Civil Engineering

Contact Number:

Kindly provide your active contact number.

Example: 0912 345 6789

STEP 3: Confirmation Option

In this section, choose whether you want to confirm or forfeit your slot for enrollment in your chosen program. Please select an option depending on your choice.

STEP 4: Submission of Pre-Registration

During this part, you will be asked if you want to successfully complete and submit this form. If yes, click Submit. If not, please select Cancel and do not submit the form.

Note: In any case, if a qualified or waitlisted student-applicant mistakenly fills out and submits information on the wrong form, his or her response will be deleted.

STEP 5: Wait for further announcements regarding enrollment.

Remember: Pre-Registration for Qualified Student-Applicants will start today, June 1, 2024 until June 7, 2024 only. Vacant slots will be automatically given to waitlisted applicants next in rank.
