✅ Fully accomplished and Notarized Personal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 212, Revised 2025) and Work Experience Sheet that can be downloaded at csc.gov.ph
✅ Photocopy of Transcript of Records
✅ Photocopy of Updated Service Record or Certificate of Employment
✅ Photocopy of Certificate of Training/Seminars Attended
✅ Photocopy of Certificate of Eligibility/Rating/License ID
✅ Other documents relevant to the position applied for For submission of the hard copy of the above-mentioned requirements, kindly visit the CSPC Records and Freedom of Information Office located at the Admin Building of CSPC Main Campus. Applications with incomplete requirements will not be entertained.
