 Photocopy of Transcript of Records
 Photocopy of Updated Service Record or Certificate of Employment
 Photocopy of Certificate of Training/Seminars Attended
 Photocopy of Certificate of Eligibility/Rating/License ID
 Performance rating in the last two rating periods
 Other documents relevant to the position applied for
