2018 Search for Most Outstanding CSPC Alumni

Guidelines for Nomination and Selection of Awardees

    1. The search aims to give recognition of the meritorious services and achievements of the CSPC Alumni. For this purpose, the Screening Committee shall select an Outstanding Camarines Sur Polytechnic Colleges Alumni (to be known as the 2018 Most Outstanding CSPC Alumni) and Most Distinguished Awardees in the following areas:
      • Good Governance and Public service;
      • Engineering and Allied fields;
      • Food and Industrial technology;
      • Information and Communication Technology, Media Communication;
      • Culture Arts;
      • Medical and Public Health promotion and services;
      • Social Work;
      • Agriculture, Forestry and Fisheries;
      • Management and Human Resource Development;
      • Corporate Social Responsibility;
      • Peace and Social Cohesion;
      • Entrepreneurship, Business and Employment creation.
    2. Eligibility: Any living Alumnus/Alumna of the College who has not been convicted of any administrative, civil or criminal charge, may be nominated to be an Awardee, by his batchmate or by another person, upon due submission of a duly notarized Nomination Form with picture of the nominee outlining his personal details and achievements.
    3. Disqualification: Vice-Presidents, Deans/Directors, Associate Deans, Head of Offices and members of the Exemplar Awards Committee are disqualified from joining the search.
    4. The Nomination Form shall be properly filled-out, notarized and actually submitted to the Screening Committee via CSPC Alumni Office email cspcalumni92@gmail.com or to the Screening Committee in the Office of the Alumni Association, Camarines Sur Polytechnic Colleges Gymnasium Building, San Miguel, Nabua, Camarines Sur on or before the cut-off date of the nomination on December 15, 2018. This form could be downloaded from the CSPC Alumni Office facebook page (https://www.facebook.com/cspcofficial) and from Camarines Sur Polytechnic Colleges Website (cspc.edu.ph).
    5. Filing of the documents may follow the order shown below.

      1. COPY OF NOMINATION FORM duly notarized.
      2. Narrative Discussion (to be answered by the nominator, use English or Filipino). In a paragraph, please discuss in brief, what makes the nominee deserving of the distinction as one of the Camarines Sur Polytechnic Colleges Most Outstanding Alumni.
      3. PERSONAL QUALITIES AND PROFESSIONAL CAREER INFORMATION
        A. Educational Background A.1 List of Formal Education.List down the formal schooling attended by the nominee. Attach a photocopy of the diploma of the highest degree earned.

        A.2  Honors received for the last five (5) years as a result of outstanding performance in the field of work.

      4. CONTRIBUTION TO SERVICE AND COMMUNITY
        1. Submit the most important accomplishments highlighting the nominee’s leadership and contribution to service in the community (may include as many as deemed significant). The format should be:
          ACCOMPLISHMENT # 1, 2, 3….
          THE WRITE-UP SHOULD CONTAIN THE NATURE OF THE ACCOMPLISHMENT, WHERE AND WHEN IT WAS PERFORMED, WHO WERE THE BENEFICIARIES, THE IMPACT OF THE ACCOMPLISHMENT AND SPECIFY THE ROLE OF THE NOMINEE. GIVE AT LEAST THREE (3) NAMES OF BENEFICIARIES AND THEIR CONTACT NUMBERS.

          IMPACT TO THE COMMUNITY:
          THIS PART SHOULD PROVIDE THE INFORMATION OF THE LASTING EFFECT OF THE ACCOMPLISHMENT TO THE COMMUNITY AND THE EXTENT TO WHICH IT IMPROVED THE QUALITY OF LIFE OF THE BENEFICIARIES.

        2. Pictures and/or news clippings (if available), and other supporting documents.
      5. ANNEXES
        A. Photocopy of Diploma
        B. Certificates of Recognition or Any documents that would support awards received
    6. The Screening Committee will appreciate the content and supporting documents holistically guided by the core values of the college: scholarship, leadership, service and character. A shortlisted nominee shall be interviewed by the screening committee at the scheduled time and venue agreed upon. When deemed necessary, an interview of other individuals who can support on the claims may also be done.
    7. Criteria for Judging

25% Scholarship (refers to the nominee’s achievement which showcases his/her knowledge or learnedness in his/her field of specializations.

25% Leadership (refers to the nominee’s ability to lead a group or community in achieving a goal).
Further, it will use the following rubric:

      • 40% support and cooperation extended by the community
      • 40% achievement of the goal within the expected time
      • 20% efficiency and effectiveness in the management of the project

50% Service to Community (refers to programs/projects initiated/conducted by the nominee which contributed in improving the quality of life of the beneficiaries. Further, it will be evaluated using the following rubric:

    • 30% sustainability
    • 30% impact to the community
    • 20% number of beneficiaries
    • 20% relevance to the needs of the community
  1. FINAL DECISION: The decision of the Screening Committee on the foregoing matters shall be final and unappealable.
